The Social Services Department strives to empower patients to be independent individuals.

The Social Services Department is composed of licensed social workers. The department assists and supports the patient and their families with adjustments to the impacts of illness and/or hospitalization. They assist the patient in receiving the maximum benefits from the health care services provided, assist with needs upon discharge, as well as making appropriate referrals to other service providers in the community on behalf of the patient.

Initially, a social worker reviews the patient’s admission assessment to determine a need for services. Based on this review, the social worker consults the patient and family members to discuss any anticipated needs, questions or concerns. Other healthcare professionals may also request a social service consultation on any patient if a need is identified. Social services can also be offered in the patients home through such programs as home health and hospice.

The mission of JRMC Social Services is to improve the patient’s quality of life by increasing their ability to build on strengths and to advocate for their needs and those of the family.

SUPPORT FOR ADVANCED DIRECTIVES

An advanced directive is a legal document that allows individuals to outline decision-making for their medical care. Advanced directives are designed to ensure that an individual’s wishes are respected and followed, even if they are unable to make their own decisions. It can also appoint someone to make decisions on the individual’s behalf if they become incapacitated. They are often associated with end-of-life care, but they can also be used to plan for unexpected medical emergencies. It is recommended that everyone over the age of 18 should have an advanced directive.
