Social Services

The Social Services Department is composed of licensed social workers, licensed clinical social workers, discharge planners, and/or nurse case managers. The department assists and supports the patient and the patient’’s family with adjustments to the impacts of illness and/or hospitalization. They assists the patient in receiving the maximum benefits from the health care services provided, assist with needs upon discharge, as well as making appropriate referrals to other service providers in the community on behalf of the patient.

JRMC Social Services’ mission is to improve the patient’s quality of life, increase their opportunity to build on strengths and to advocate for their need and those of the family.

The Social Services Department strives to empower patients to be independent individuals.

 

Social Services include, but are not limited to:

  • support for the patient and their family
  • casework services
  • discharge planning
  • long-term care and rehabilitation arrangement
  • home health referral
  • hospice
  • education on community resources and services
  • financial planning/referral
  • consultation/counseling
  • program development

 

Initially, a social services professional reviews the patients admission assessment to determine a need for services. Based on this review, the social services staff member consults the patient and family members to discuss any anticipated needs, questions or concerns. Other healthcare professionals may also request a social service consultation on any patient if a need is identified. Social services can also be offered in the patients home through such programs as home health and hospice.
